By law, the Social Security Administration (SSA) is required to periodically conduct “Continued Disability Notifications” for all persons receiving disability benefits from this agency. SSA will at any time notify any recipient per. post that it should initiate a review. This process requires recipients to complete a Continuing Disability Review package that SSA sends to recipients to help SSA update information about the recipient’s medical conditions and recent treatments.

In addition, SSA offers an online option for recipients to complete the disability update form and provide any supporting documents about their medical treatment or work activities.

The Social Security Administration has designed this form with convenience in mind – and to save you time. People can access the online form at www.ssa.gov/ssa455-online-form. (Use either Microsoft Edge or Google Chrome for the best online experience.)

Once the Continuing Disability Review form is completed, you must add your CPR number, current address and telephone number as well as a valid email address to complete it. Persons must also have received a request for an updated disability report by mail.

When individuals click on “Click to sign”, they will receive an email from echosign.comwho will ask for confirmation of one’s digital signature. Check your junk folder if you do not receive it within a few minutes. If the signature is not complete, the form will not be processed.

Due to the COVID-19 pandemic, social security offices could not receive visitors except for previously arranged appointments in particularly limited critical situations. However, SSA continues to provide its services via telephone and internet. If there are questions about social benefits and services, you need access www.socialsecurity.gov.
